Backed by the Centers for Medicare & Medicaid Services, the GUIDE Model (Guiding an Improved Dementia Experience) delivers expert care, compassionate support, and practical resources — all tailored to help you and your loved ones live more comfortably and confidently.
What you can expect from the GUIDE Model:
Enhanced Nursing Services
A dedicated nurse from Curana Health will reach out to you and your loved one once a month to check in on their overall health and how you’re feeling about their care. They will also serve as your contact for health and wellness needs in between check-ins.
Best-in-Class Education Platform
Curana Health has partnered with ianacare to offer an online tool that features a caregiver community, a content library, interactive sessions, and more.
Care Plans Made Just for You
We don’t believe in one-size-fits-all care. Your nurse will work with you to build a personalized care plan that reflects your loved one’s specific diagnosis, needs, and preferences.
Respite Care Coverage
You may qualify for up to $2,500 per year for respite care services (for those eligible).

We will start by taking time to learn about your loved one’s specific diagnosis and needs. Then, a specially trained nurse will lead the creation of a care plan that addresses both their condition and personal preferences.

Part of the National Plan to Address Alzheimer’s Disease, the GUIDE Model is only available through certain Medical Groups known to provide exceptional dementia care. Curana Health is one of approximately 400 selected participants across the country.
